additionally, we have the big google map embed now, maybe go full width with it at the top, with the fields/text following, kinda like the realtree site did.
i mocked up the content we need in psd/screencap above. lets just use the two col layout we using on other subpages. above is crappy arial font or something, play with making copy nicer/using fonts we are using on other subpages, mix of all caps/regular case, maybe serif on some stuff
on the fields, i just copy/pasted the realtree form. make the fields be not rounded corners, match the grey of the field bg to the grey of the main nav highlight box, match the field copy color to other dark copy we are using on cccd. make the send msg box be red plate with wht txt, left alight it instead of centered as is

were using php 5.3 on cccd and 5.5 on realtree. what did you do to update it. getting an error on the return message after sending form. looks to be a version issue.
